  2. Fill out the form on your computer and email it to us, or print it out and fax it in. Detailed instructions are included on the order form, and you can also phone in your order.
  3. Most of our retailers are based in the U.S. and Canada, but we are interested in hearing from you if you want to offer our products elsewhere.
  4. When we receive your order, we may need you to provide a copy of your business license and/or proof of your retail presence.
